About The Position

The Medication Therapy Management (MTM) Pharmacy Technician supports pharmacists in delivering comprehensive medication management services to patients. This role is responsible for patient outreach, medication reconciliation, documentation, adherence monitoring, and coordination with healthcare providers, insurance plans, and pharmacy staff to improve medication outcomes and support quality initiatives. The MTM Pharmacy Technician plays a key role in ensuring timely completion of MTM services while maintaining compliance with HIPAA regulations, Medicare Part D requirements, and company policies.

  • Previous Long Term Care Pharmacy experience
  • One to three years of pharmacy technician experience
  • Proficiency with information systems
  • Microsoft Office

  • Active Tennessee Board of Pharmacy Registration
  • High school diploma or general education degree (GED)
  • Working safely and follow safety rules
  • Report unsafe working conditions and behavior
  • Take reasonable and prudent actions to prevent others from engaging in unsafe practices

Nice To Haves

  • Pharmacy Technician Certification (CPhT) preferred
  • Completion of pharmacy technician program preferred
  • Or equivalent combination of education and/or experience

Responsibilities

  • Identify patients who qualify for Medication Therapy Management services and schedule appointments with providers and residents for medication reviews.
  • Gather comprehensive medication lists, including prescriptions and OTC products, through patient interviews and provider records.
  • Contact patients to review medication usage, discuss adherence, and address potential barriers to taking medication.
  • Document medication therapy problems, interventions, and patient conversations in MTM software (e.g., OutcomesMTM) for pharmacist review.
  • Perform follow-up calls to patients regarding therapy changes, lab tests (e.g., for anticoagulation), or medication refills.
