About the position
The Medical Social Worker (MSW) role at Interim HealthCare involves providing home-based social services to patients facing health challenges. The position focuses on connecting patients and families with necessary resources, counseling, and support to help them manage their conditions effectively. This role emphasizes advocacy, collaboration with healthcare teams, and the delivery of compassionate care to improve patient outcomes.
Responsibilities
- Provide home-based social services to patients dealing with depression, anxiety, poor nutrition, or a lack of resources.
- Work as part of a home health team, collaborating with RNs, LPNs, CNAs, HHAs, PTs, OTs, or SLPs to focus on the patient's plan of care and individual goals.
- Monitor a patient's psychosocial condition and identify social and emotional needs.
- Conduct patient assessments, document progress, and ensure the patient is moving toward goals.
- Provide counseling, community resource planning, crisis intervention, and advocacy.
- Consult with family and caregivers on the patient's plan of care and how to help them progress.
Requirements
- Master's degree in Social Work and active MSW license in IL.
- Minimum of 1 year of social work experience, ideally in home healthcare.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Strong interpersonal skills, good communicator, empathetic, compassionate, and resourceful.
