About the position

The Per Diem Medical Social Worker position at Advancare Healthcare Services involves providing compassionate home health care to patients in the Chicagoland area. This role focuses on assessing patients' needs, implementing care plans, and collaborating with an interdisciplinary team to ensure comprehensive support. The position is part-time and operates on a pay-per-visit compensation model, emphasizing quality care and a supportive work environment.

Responsibilities

• Assessing and evaluating patients for community resources and in-home support services.
• Assisting in the implementation of the plan of care.
• Supporting patients with advance directives.
• Preparing and submitting accurate clinical notes.
• Completing all charting inside the patient's home using the electronic medical records system.
• Collaborating with the interdisciplinary team to provide comprehensive care.
• Participating in discharge planning and in-service education programs.

Requirements

• Master's Degree from a program accredited by the Council on Social Work Education.
• Certified/licensed and in good standing within the state, preferably with an LCSW license.
• Minimum of one year of experience as a Medical Social Worker, preferably in a healthcare setting.
• Experience in home health is preferred.
• Basic Life Support Certification (BLS) required.
• Excellent communication skills and ability to follow written instructions.
• Self-directed and able to work with little direct supervision.
• Demonstrates tact, patience, and good personal hygiene.
• Valid driver's license and insured automobile.
