About the position

The Medical Social Worker for the Exceptional Family Member Program at David Grant Medical Center (DGMC), Travis AFB, CA, will perform duties related to care coordination, discharge planning, and case management in a medical setting. The position requires a Master's degree in Social Work (MSW) from an accredited institution and a current, unrestricted license to practice as a Licensed Clinical Social Worker. The role involves working with service members and their families, addressing a variety of psychosocial issues and ensuring compliance with medical privacy standards.

Responsibilities

• Apply Special Needs Coordinator functions effectively.
• Demonstrate knowledge of psychosocial practice theories and principles.
• Conduct individual, family, group, and community assessments.
• Plan, implement, and evaluate programs.
• Communicate effectively with patients, families, and medical staff.
• Resolve complicated healthcare, social, interpersonal, and financial patient situations.
• Maintain knowledge of medical privacy and confidentiality standards.

Requirements

• Master's degree in Social Work (MSW).
• Graduate from a School of Social Work accredited by the Council on Social Work Education (CSWE).
• Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker/MSW.
• Minimum of two years full-time experience in care coordination, discharge planning, or case management in a medical setting.
• Moderate computer literacy, familiar with MS Office, Adobe PDF, and general database use.
• Strong organizational, problem-solving, and communication skills.
