About the position
The MDS Nurse RN at The Laurels of Chatham is a clinical role focused on patient care management without the need for bedside nursing. This position requires strong organizational skills and the ability to work autonomously while conducting assessments and coordinating care plans for residents. The role is integral to the RAI process and involves collaboration with various healthcare professionals to ensure comprehensive care for residents.
Responsibilities
- Completes the MDS, CAA's and care plans within regulated time frames.
- Assesses residents through physical assessment, interview, and chart review.
- Discusses resident care needs with caregivers, including physicians, nursing, social services, therapy, dietary, and activity staff.
- Reviews information from hospitals, consults, and outside agencies to complete assessments and care planning.
- Coordinates, identifies, and/or initiates significant change MDS'.
- Conducts PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
- Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.
Requirements
- Registered Nurse (RN) license is required.
- RAC-CT certification is preferred.
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) is strongly preferred.
- Experience as an MDS Nurse is preferred.
Nice-to-haves
