About the position

The MAT Case Manager at Terros Health is responsible for working with individuals diagnosed with opiate use disorders enrolled in medication-assisted treatment (MAT). This role involves engaging clients through clinical interventions, motivational interviewing, and supportive listening to promote recovery and improve quality of life. The position emphasizes collaboration with an interdisciplinary clinical team and requires effective communication with clients and their support systems.

Responsibilities

  • Engage, educate, and coordinate care with clients receiving medication assistance services for the MAT program.
  • Conduct outreach and follow-up services, including crisis interaction and missed appointments.
  • Ensure ongoing collaboration and communication of clinical information with involved parties.
  • Participate in clinical staffing, case conferences, and meetings with clients and their families.
  • Provide excellent customer service to clients and stakeholders.

Requirements

  • High School Diploma or GED required; Bachelor's degree in behavioral health preferred.
  • Behavioral health experience or a Master's degree in the field of behavioral health preferred.
  • Interpersonal skills for positive relationships with co-workers and clients.
  • Excellent oral and written communication skills.
  • Basic understanding of DSM-5 and experience with opiate use disorder patients preferred.
  • Proficient in Microsoft Office Suite and experience with electronic medical records.

Nice-to-haves

  • Experience in crisis intervention and case management.
  • Knowledge of community resources for behavioral health support.
