About the position

The Licensed Vocational Nurse (LVN) at ConcertoCare plays a crucial role in delivering clinical care to patients in their homes and providing virtual clinical support. This hybrid position involves both in-home visits and virtual care facilitation, ensuring that patients receive timely and appropriate care. The LVN is responsible for direct patient care, health screenings, clinical procedures, and care coordination, all under the supervision of authorized healthcare practitioners.

Responsibilities

• Provide direct nursing care to patients through routine and urgent in-home and virtual visits under the direction of an authorized health care practitioner.
• Facilitate virtual visits in the patient's home to support care delivery under the direction of a remote clinical practitioner.
• Provide support for clinical visits, including chart preparation, clinical intake, after-visit care coordination, and clinical documentation and order entry support.
• Coordinate and support patient care by implementing care plans and performing outbound calls and tasks as directed by a clinical supervisor.
• Process and track orders and referrals placed by providers, ensuring patients complete them according to their treatment plan.
• Engage patients in their healthcare journey, establishing meaningful connections and providing tailored care.
• Deliver exceptional customer service to patients, caregivers, and clinical partners with proactive communication.
• Perform clinical skills and procedures such as blood draws, medication administration, wound care, and IV insertion under supervision.
• Document all patient care support activities accurately and timely in the medical record.
• Participate actively in team huddles and organizational meetings as requested.
• Adhere to compliance policies, procedures, and standards of conduct.

Requirements

• Hold an active unrestricted LVN license in the state of California.
• 2+ years clinical experience required, with urgent care, emergency room, or community home health experience preferred.
• IV or blood draw certification or relevant work experience.
• Current certification in Basic Life Support (BLS) required.
• Ability to conduct independent patient home visits.
• Ability to travel 75%+ of the time within an assigned area.
• Average to advanced computer and software skills required.
• Proof of liability and property damage insurance on the vehicle used is required.
