About the position

The Healthcare Coordinator plays a crucial role in supporting patients with high-risk health conditions as they navigate the healthcare system. This position focuses on patient empowerment through education, resource provision, and advocacy, ensuring a high quality of life for patients and their families. The Healthcare Coordinator collaborates with multidisciplinary care teams to facilitate transitions of care and acts as a resource for clinic staff, performing nursing duties within the scope of a LVN/LPN license.

Responsibilities

  • Identify patients at high risk through transitions of care to reduce utilizations, including ER visits and hospital admissions.
  • Support longitudinal care of patients with chronic conditions by performing health assessments and initiating medication reconciliation.
  • Conduct Motivational Interviewing and Self-Management Goal setting to empower patients.
  • Provide patient education in a self-management format.
  • Assist with the transition of patients from inpatient to outpatient settings, ensuring follow-up with PCP post-discharge.
  • Coordinate with providers to establish or update plans of care.
  • Perform accurate and timely documentation in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare accurate and timely reports for weekly meetings.
  • Maintain continued competence in nursing practice and knowledge of current evidence-based practices.

Requirements

  • Licensed Practical/Vocational Nurse with an active and unrestricted license to practice in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
  • Proficient computer skills to work efficiently with electronic medical records.
  • Proven excellent verbal and written communication skills.
  • Proven excellent organizational and prioritization skills.
  • Proven solid interpersonal skills.
  • Proven ability to interact productively with individuals and multidisciplinary teams.

Nice-to-haves

  • IV Certification
  • Experience related to patient education and/or motivational interviewing skills and self-management goal setting.
  • Experience with outbound patient telecommunications.
  • Fluent written and verbal skills in English and Spanish.
