About the position

The LTSS Service Coordinator-Clinician (Case Manager - LCSW) is responsible for the overall management of a member's case, contributing to the LTSS care coordination process. This role involves conducting assessments, managing member needs across various health domains, and coordinating care for members with chronic illnesses and disabilities. The position requires travel up to 60% of the time across several counties in North Carolina, and the work schedule is Monday to Friday from 8:30 AM to 5 PM EST.

Responsibilities

• Identifies members for high risk complications.
• Obtains clinical data.
• Identifies members that would benefit from an alternative level of care or other waiver programs.
• Verifies and interprets information, conducts additional assessments as necessary, and develops, monitors, evaluates, and revises the member's care plan.
• Participates in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities.

Requirements

• Requires an LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with applicable state law and Nursing Diploma or AS in Nursing or a related field.
• Minimum of 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.

Nice-to-haves

• LCSW strongly preferred.
• Case management or home health experience preferred.
• Proficiency in Microsoft Office Suite products preferred (Outlook, Word, Excel).
• MA/MS in Health preferred.
• Valid driver's license strongly preferred.
