About the position

The Licensed Practical Nurse (LPN) position at Charles T. Sitrin Health Care Center involves providing compassionate and professional nursing care in a long-term care setting. The LPN will be responsible for supervising Nurse Aides, administering medications, monitoring residents' conditions, and ensuring high standards of care are maintained. This role requires effective communication and leadership skills, as well as adherence to ethical and legal standards in nursing practice.

Responsibilities

  • Responsible for the overall functioning of assigned house during shift including supervisory direction to Nurse Aides.
  • Establishes work assignments for unit Nurse Aides and supervises to assure thorough task completion.
  • Reports to acting Supervisor data that needs coordination with other disciplines.
  • Responsible for the timely and accurate administration and documentation of physician ordered medications and/or treatments to assigned residents.
  • Maintains facility procedure for the ordering and receiving of physician prescribed medications.
  • Monitor for changes in residents' conditions with appropriate chart documentation and refers problem areas to acting Supervisor.
  • Documents in electronic medical records in a timely manner according to facility policy and procedures.
  • Provides shift input of resident care needs to the Interdisciplinary Care Plan for continuity of resident care.
  • Maintains awareness of changing resident needs, both physical and emotional, and adjusts work assignments accordingly.
  • Maintains documentation of shift occurrences to promote thorough reporting of resident activity to oncoming shift.
  • Reports changes in residents' condition to Charge Nurse or Supervisor.
  • Consults with disciplinary team in establishing information, notification to families, physicians and/or administration.
  • Proactive in preventative/protective skin care program.
  • Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.
  • Assures completion of monthly reports as requested by Nursing Administration.

Requirements

  • Graduate of an accredited School of Practical Nursing.
  • Current Licensure as a Licensed Practical Nurse in New York State.
  • Leadership ability and effective communication skills.
  • Demonstrates competency by completing annual job specific skills of unit.

Benefits

  • Full-time sign-on bonus of $2,500
  • Part-time sign-on bonus of $1,500
  • Pay rate of $22.75 - $30.25 based on experience
  • Eligible for benefits upon hire
