- Communicate effectively with healthcare teams and families to ensure continuity of care.
Requirements
- Active LPN license in state of practice.
- 1-2 years of experience in home health or similar setting.
- Strong observational, communication, and documentation skills.
- Ability to work independently with a compassionate approach.
