This is a Private Duty LPN role in Warsaw, IN, with a 3-day per week schedule, focusing on one patient for consistent, lower-stress care compared to facility work. You will provide 1:1 nursing care in the home, follow the plan of care, monitor patient condition, document care, and communicate with the care team. We offer strong clinical support and more control over your day.
Requirements
- Active LPN license
- 1+ year of nursing experience
- Strong observation and communication skills
- Reliable transportation
- Ability to assist with patient mobility, up to 50 pounds
Responsibilities
- Provide 1:1 nursing care in the home
- Follow the plan of care and physician orders
- Monitor patient condition and report changes
- Document care accurately and on time
- Communicate with RN, physician, and care team
