About the position

The LPN (Pharm) position at St. Francis Community Health Services involves providing nursing care under the supervision of Registered Nurses in a hospice inpatient setting. The role focuses on administering routine services, medications, and ensuring patient safety while adhering to the hospice's philosophy and policies. The LPN will be part of an interdisciplinary team, participating in patient admissions, transfers, and discharges, and will be responsible for documenting patient observations and care changes.

Responsibilities

• Administer routine services in caring for hospice patients under the supervision of Registered Nurses.
• Participate in the admission, transfer, and discharge of nursing care as a member of the Interdisciplinary Team.
• Report and document all observed medication errors, adverse drug reactions, and unusual occurrences.
• Provide safety measures for patients, including side rails and fall risk protocols.
• Maintain a clean and comfortable environment for patients, considering cleanliness, ventilation, and lighting.
• Communicate effectively the hospice's philosophy, goals, objectives, and policies to patients, families, physicians, and staff.

Requirements

• Completion of an accredited LPN Program.
• State of Hawaii LPN License.
• Minimum one (1) year previous experience in nursing.
• Current Healthcare Provider CPR certification.

Nice-to-haves

• Acute Care/Oncology Experience preferred.
• Experience in Skilled Nursing Facility (SNF) or Hospice Preceptorship.
