Our LPN's deliver various personal care and services in accordance with an established plan of care and provide the personal needs and comfort of patients in their homes. In this role, you will work with our clients in their homes, helping them with basic personal hygiene and domestic tasks as needed, according to their requirements and limitations. You will provide compassionate care and assistance and document clients' progress to help ensure their overall wellbeing. Requirements

Active LPN License
At least 1 year of nursing experience in a healthcare setting
Valid Driver’s License or State-Issued ID
Current CPR/BLS Certification
TB Test Results
Social Security Card
Vaccination Card
Fingerprinting / Background Check Clearance

3–5 years of nursing experience in a healthcare setting

Responsibilities

Deliver various personal care and services in accordance with an established plan of care.
Provide the personal needs and comfort of patients in their homes.
Help clients with basic personal hygiene and domestic tasks as needed, according to their requirements and limitations.
Provide compassionate care and assistance.
Document clients' progress to help ensure their overall wellbeing.
Perform breathing treatments and ongoing respiratory assessments/monitoring.
Provide preventative skin care assistance to help maintain skin.
Assist with transfers and mobility as needed.
Provide incontinence care and personal hygiene support.
Administer bolus tube feedings through the PEG tube.
Perform routine PEG tube changes every three months.
Accompaniment and assistance during outings and appointments.
