Requirements
- Current LPN license in Connecticut
- CPR certification
- Strong communication and organizational skills
- Ability to maintain accurate documentation and work in a fast-paced environment
Nice To Haves
- Experience in geriatric or rehabilitation nursing
- Prior supervisory experience
Responsibilities
- Leading and supervising nursing staff to ensure excellent care delivery
- Providing hands-on nursing care based on individualized resident care plans
- Monitoring resident conditions and communicating changes with families and providers
- Completing accurate assessments, documentation, and shift reports
- Conducting rounds to ensure a safe, clean, and supportive environment
- Responding promptly to emergencies and resident needs
- Supporting admissions, discharges, and care plan development
