About the position

The LPN Nurse / Medical Records position at South Haven Health And Rehabilitation LLC involves working closely with patients to support their recovery journey. This role is not in a hospital setting, allowing for the development of meaningful relationships with patients as they regain independence. The LPN will be responsible for managing medical records, ensuring accuracy and completeness, and supporting the nursing team under the direction of the Director of Nursing.

Responsibilities

• Prepare medical records upon admission and maintain them until discharge.
• Enter resident information into the computer system for new admissions.
• Ensure all necessary forms are present for new admissions.
• Review records, MARS, and TARS for accurate transcription and enter any orders into the computer system.
• Reduce records monthly, file information in office files, and retrieve files as needed.
• Compile discharge records, close records, and file information appropriately.
• Perform record audits for completeness, ensuring all documents are present and in order.
• Ensure physician completes and signs discharge summaries.

Requirements

• Must be a Licensed Practical Nurse (LPN).
• Experience in long-term care is a plus.
• Working knowledge of medical terminology and physician orders.
• Excellent telephone, communication, and interpersonal skills.
• Good organizational and filing skills.
• Basic computer skills required for electronic documentation.
• Working knowledge of computers, keyboard, and data processing skills.
• Patience, tact, enthusiasm, and a positive attitude toward the elderly and disabled.
• Must be able to speak, read, write, and understand English.
• Must be at least 18 years of age.
