About the position
Ardor Health Solutions is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for a travel position in a home health setting in Newport Center, VT. This full-time role involves providing care to patients in their homes and requires an active Vermont home health license. The position is set to start on November 25, 2024, and will last for 13 weeks, with a commitment of 32 hours per week on a 5×8 days shift schedule.
Responsibilities
- Provide nursing care to patients in a home health setting.
- Administer medications and treatments as prescribed.
- Monitor patient health and report any changes to the healthcare team.
- Educate patients and their families on health management and care plans.
- Maintain accurate patient records and documentation.
Requirements
- Active Vermont home health license.
- Minimum of 2 years of experience as a Licensed Practical/Vocational Nurse.
