About the position

The Licensed Practical Nurse (LPN) Home Health position at Alternate Solutions Health Network involves providing compassionate, home-based nursing care to patients as part of an interdisciplinary team. The role emphasizes patient care, education, and monitoring, ensuring positive health outcomes while working autonomously in a flexible schedule environment.

Responsibilities

• Provide skilled nursing care in a patient's home as directed by the attending physician.
• Observe and monitor patient conditions, notifying the case manager of any changes.
• Administer medication as prescribed and educate patients on preventative measures to minimize re-hospitalizations.
• Educate patients and families on the disease process using teach-back methods to ensure understanding.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Understand and follow agency policies, procedures, rules, and regulations.
• Attend in-service training and mandatory agency meetings.

Requirements

• Licensed Practical Nurse (LPN) with current license in the state of employment.
• Minimum one-year experience as an LPN in an acute care setting.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
