About the position

The LPN position at the Hospice Care Center is a per diem role responsible for providing direct patient care and medication administration under the supervision of a registered nurse (RN). The LPN will assess patients' statuses, collect and analyze medical data, and ensure compliance with federal and state regulations. This role requires flexibility to work day and night shifts as needed, focusing on delivering compassionate care to patients and their families in a hospice setting.

Responsibilities

  • Collects and analyzes data from patient's medical history, nursing assessments, and staff input on an ongoing basis.
  • Observes patient's condition and response to treatment/medications, recommending changes to the patient's Plan of Care.
  • Prepares complete and accurate documentation in accordance with all HOSF policies and procedures, Federal/State regulations, and Medicare/Medicaid guidelines.
  • Provides direct patient care according to all professional and regulatory standards.
  • Maintains strict confidentiality and respect for patient/family privacy and possessions.
  • Practices Universal Precautions and safety at all times.
  • Teaches patient/family skills needed to maintain patient comfort and perform personal hygiene activities.
  • Provides emotional support to patients, family, and other caregivers.
  • Participates in performance improvement and quality control activities.
  • Increases knowledge of palliative care through active participation in educational opportunities.
  • Assists patients/families with understanding requirements during implementation of a disaster plan.
  • Attends HOSF General Staff Meetings, nurses meetings, and educational training sessions as scheduled by the Care Center Manager.

Requirements

  • Valid, current Florida LPN license
  • Valid Florida driver's license
  • Minimum three years clinical experience, preferably in hospice inpatient setting, home health, oncology, acute care, or long-term care
  • Ability to read, write, and speak English; bilingual preferred
  • Excellent communication and interpersonal skills
  • Ability to manage time and triage phone calls from patients, families, and physicians
  • Ability to lift fifty (50) pounds without difficulty
  • Good visual and audio acuity
  • Able to work under varying degrees of stress

Nice-to-haves
