About the position

The LPN Evening position at Bethel Lutheran Home involves providing comprehensive nursing care to residents in the Long Term Care area. The role requires the nurse to demonstrate clinical judgment, critical thinking, and effective coordination of resident care while adhering to nursing standards. The LPN will be responsible for assessing, planning, implementing, and evaluating resident care, ensuring a safe and therapeutic environment, and supporting the overall mission of the facility.

Responsibilities

  • Utilizes the nursing process to assess, plan, implement, and evaluate resident care.
  • Performs regular resident assessments to identify specific needs.
  • Demonstrates knowledge and skills in the nursing process for residents requiring professional nursing expertise.
  • Conducts geriatric assessments and treatments.
  • Identifies and reports suspected abuse and neglect cases.
  • Assesses geriatric safety issues and demonstrates nursing care competencies.
  • Interprets resident data relevant to the geriatric population.
  • Documents nursing diagnoses, expected outcomes, and interventions in the care plan.
  • Monitors consistency between the resident care plan and medical staff directions.
  • Provides a safe, comfortable, and therapeutic environment for residents.
  • Initiates and evaluates resident and family education.
  • Revises care plans based on evaluations and changes in medical plans.
  • Documents resident responses to nursing interventions.
  • Uses clinical judgment to meet resident care needs and establish priorities.
  • Monitors safety and effectiveness of the environment and equipment.
  • Participates in activities to facilitate good interpersonal communications within the facility.

Requirements

  • Graduate of an accredited school of Nursing with valid SD State licensure as an RN or LPN.
  • American Heart Association Basic Life Support certification required.
  • One year prior professional nursing experience preferred.
  • Ability to demonstrate self-confidence and a positive attitude.
  • Excellent team player with strong communication skills.
