This position is full-time, evenings from 3pm to 11pm, with a starting salary of $33.00/hour. The role involves assisting the Resident Care Manager (RCM) with the supervision of the Resident Care Aides (RCA), medication management assistance, activities of daily living, and the observation, evaluation, and implementation of the medical/clinical needs of the residents.

Responsibilities

  • Support staff development and assist with completion and documentation of annual RCA observations, training and in-services as directed and developed by the Resident Care Manager.
  • Supervise the delivery of services and the nursing staff delivering the services for the assisted living, enhanced assisted living and special needs assisted living residents.
  • Observe and evaluate the medical/clinical, physical, emotional and cognitive needs of the residents with notification of changes to the medical provider and resident representative.
  • Ensure completion and documentation of scheduled daily treatments.
  • Transcribe medical provider orders to the EMR ensuring implementation and appropriate documentation per NYS DOH regulations and ESLC Assisted Living Policies.
  • Communicate resident changes and functional needs to nursing staff members with updates to the Care Card and Individual Service Plan, as needed.
  • Ensure medication management assistance by RCAs is completed per NYS DOH regulations and ESLC Assisted Living Medication Policies.
  • Oversee ADL completion by RCAs per Individual Service Plan and ESLC Assisted Living Policies.
  • Assign nursing staff duties, tasks and other responsibilities.
  • Monitor and review staffing coverage in conjunction with the Nursing Services Coordinator.
  • Provide resident and staff teaching in collaboration with the Director of Quality and Education.
  • Actively participate in the Interdisciplinary Team meetings.
  • Participate in facility safety and quality assurance processes.
  • Assist with management of medication assistance.
  • Perform the job responsibilities of the RCA and/or LPN, as needed.
  • Maintain medical/clinical documentation and resident records as required.
  • Complete on-call duty on a rotational basis to meet the Nursing Department needs.

Requirements

  • Nursing skill experience.
  • Effective written and verbal communication skills.
  • Must have effective social and communication skills, need to be flexible and willing to adapt to change.
  • Ability to interact courteously and tactfully with staff, residents, family members, visitors, vendors, and the public.
  • Basic computer skills preferred.
  • Current New York State license and registration as a licensed practical nurse.
  • Three years’ experience with geriatric population.
