About the position
A&C Private Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to join their team. The LPN will be responsible for conducting supervisory visits for clients, monitoring vital signs, and ensuring the overall health and well-being of clients in their homes. This role is crucial in providing personalized care and support to help clients maintain their independence and quality of life.
Responsibilities
- Monitoring and recording client's vital signs as needed, reporting abnormal readings to the RN supervisor.
- Observing client's functional level; reporting any changes in the client's physical condition and health care needs to the supervising RN.
- Conducting Supervisory Review for non-medical frail clients.
- Assisting clients with medications; observing for possible reactions; reporting any reactions to the supervising RN.
- Preparing progress notes and supervisory visit entries, documenting client's status.
- Teaching/Coaching self-care activities to clients, client representatives, and caregivers.
- Assisting with and/or providing client care as needed.
- Reviewing, signing, and dating all clinical record entries made by the personal support aides supervised by the LPN.
- Collaborating with the supervising RN to update client care plans.
- Completing a face-to-face supervisory visit with assigned clients and, when appropriate, the client's representative and/or caregiver, in the client's home.
- Notifying the supervising RN of any significant changes in client's status.
- Completing an in-home supervisory visit with assigned clients at least once per year to observe and monitor the in-home performance of the personal support aides.
- Completing timely, pertinent supervisory notes that include the documentation.
Requirements
- Valid LPN license in the state of Georgia.
- Experience in patient monitoring and home care services.
- Strong communication skills to report client status to RN supervisors.
- Ability to document and maintain accurate client records.

- Knowledge of medication management and potential reactions.
