About the position
The Medical Social Worker position is a part-time role focused on providing hospice care through home visits. The role involves working with patients and their families to address social and emotional factors affecting health, contributing to care plans, and offering bereavement support. The position offers flexibility in scheduling, with a preference for morning or business hours, and is open to PRN opportunities in the Cape May Court House and Egg Harbor areas.
Responsibilities
- Provide medical social services under the direction of a physician and Interdisciplinary Group.
- Assist patients and families in understanding significant personal, emotional, environmental, and social factors related to health status.
- Contribute to the development of a comprehensive, integrated Plan of Care for patients.
- Instruct health care team members on community resources available to assist patients.
- Function as Bereavement Coordinator and supervise bereavement services as needed.
- Establish a Plan of Care addressing bereavement needs for up to thirteen months following the patient's death.
Requirements
- Master's degree in social work or related field accredited by the Council on Social Work Education.
- Licensed as a Social Worker in NJ.
- One year of social work experience in a health care setting.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
