Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. This is a limited duration position, anticipated until August, with the opportunity for extension and full-time hire. The role is full-time, with day shifts from 8:30 AM to 5:00 PM, Monday through Friday. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. The Kaiser Intensive Community Treatment (ICT) program, where this role is situated, is an Assertive Community Treatment (ACT) program serving Clients with Serious Mental Illness (SMI). The program is designed to meet the needs of Kaiser members who are high users of inpatient psychiatric services or who are at high risk of acute psychiatric hospitalizations. Teams work closely with Kaiser’s Psychiatrists and Psychologists to ensure best-of-class treatment and support services, with all services provided in the community.
