About the position

The Licensed Social Worker (LMSW) - Homecare position at Hartford HealthCare involves providing personalized care to patients in their homes, focusing on their short and long-term needs. The role requires collaboration with patients, families, and healthcare teams to address bio-psychosocial needs and implement care plans. The Social Worker will utilize their clinical skills to conduct assessments, provide counseling, and advocate for patients, ensuring they receive the necessary support and resources.

Responsibilities

  • Collaborate with the interdisciplinary team regarding patients identified for Social Work Services.
  • Complete comprehensive in-home psychosocial evaluations considering various factors such as developmental, social, and medical issues.
  • Utilize evidence-based assessment tools to determine cognitive status and behavioral health needs.
  • Determine appropriate levels of care including needs for skilled-nursing facilities and assisted living.
  • Create care plans with measurable goals based on comprehensive assessments.
  • Implement short-term interventions to address identified barriers and promote health.
  • Provide clinical treatment services including triage, crisis intervention, and therapy.
  • Utilize Motivational Interviewing and Coaching skills to assist patients and families in identifying goals.
  • Educate patients, families, and the community regarding disease prevention and health maintenance.
  • Advocate for patients' needs to improve access to care and service delivery.
  • Monitor and alter care plan goals and interventions as necessary.
  • Maintain timely documentation of social work services reflecting patient information and outcomes.
  • Facilitate team-based care and collaboration with healthcare team members.
  • Participate in case conferences and interdisciplinary team meetings.
  • Act as a consultant regarding abuse, neglect, and community resources.
  • Conduct outreach activities for program marketing and community education.

Requirements

  • 2-3 years of healthcare experience, including acute and community health experience.
  • LMSW licensure required.
  • Considerable knowledge of social, cultural, economic, and psychological issues influencing patient behavior.
  • Familiarity with evidence-based assessment tools such as PHQ-9, SLUMS, and others.
  • Proven ability to work effectively both independently and in a team environment.
  • Excellent written and verbal communication skills.
  • Strong organization skills and attention to detail.
  • Strong computer skills, including Microsoft Office; Epic experience is a plus.
