About the position

The Licensed Practice Nurse (LPN) position at TriHealth involves providing both direct and indirect patient care in a primary care setting. The LPN will work closely with care delivery providers to identify gaps in patient care, schedule necessary appointments, and follow up on referrals. This role includes pre-visit planning, managing patient communications through electronic portals, and assisting in transitions of care. The LPN is expected to participate in process improvements and support the clinical goals of the practice, ensuring high levels of patient satisfaction and engagement.

Responsibilities

  • Coordinates the primary care rooming process and relevant medical procedures for adult and pediatric patients.
  • Performs immunizations, venipuncture, point of care testing, and retinal scan imaging.
  • Documents all facets of care accurately, including clinical calls, patient rooming questions, and order entries.
  • Addresses patient messages in a timely manner and escalates issues as necessary.
  • Utilizes MyChart messaging to enhance patient communication.
  • Participates in patient-centered medical home team activities during patient visits, reviewing charts for clinical gaps in care.
  • Assists with outreach campaigns to close gaps in care and supports pre-visit planning.
  • Informs physicians of any potential barriers identified by patients.
  • Understands population health and value-based contracts, utilizing metrics for wellness and chronic disease management.
  • Completes post-ED/post-inpatient discharge outreach for identified risk patient groups.
  • Collaborates with the care team, including Complex Care RN and Social Worker, to update care documentation.
  • Provides education and coaching on wellness and chronic disease management, such as Diabetes Education and Colon Cancer Screening.

Requirements

  • Graduate of an approved technical, professional, or vocational program in Healthcare.
  • Healthcare clinical experience preferred in a physician practice or related field.
  • Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification required.
  • Knowledge of medical office flow, especially clerical/front office tasks.
  • Ability to make quick decisions based on well thought out consequences/results.
  • Knowledge of Electronic Medical Records (EMR) systems.
  • Experience with practice management software and medical coding/billing is strongly encouraged.
  • 3-4 years of clinical healthcare experience.

  • Experience with multiple age groups and understanding of recommended screenings based on age.
  • Understanding of chronic disease management processes.
  • Experience with patient-centered medical home models.
