The Licensed Practical Nurse (LPN) will be responsible for providing direct care to residents as needed in accordance with care plans, policies and procedures, and nursing standards of care. This role involves admitting, transferring, and discharging residents, as well as regularly checking on residents to observe and evaluate their physical and emotional status. The LPN will also direct Nursing/Resident Care Assistants in their responsibilities and report any concerns to the Resident Care Coordinator. Documentation according to policies and procedures and regulations is essential, including charting nurse's notes in an informative, descriptive, and legible manner that reflects the care provided and the residents' responses to care and intervention. The LPN will report discrepancies, problems, and issues regarding resident care, transcribe and check physician orders, administer medications, complete assessments on assigned residents, and communicate pertinent information to physicians, residents, families, and nursing staff. Additionally, the LPN will perform other duties as assigned.
