About the position

Three Oaks Health Services is seeking a Licensed Practical Nurse (LPN) to join our team. As a member of the North Shore Healthcare family, you will be part of the largest post-acute provider in the Upper Midwest, with 70 skilled nursing and assisted living centers across Wisconsin, Minnesota, Michigan, and North Dakota. In this role, you will work under the supervision of the RN Unit Manager or Director of Nursing (DON) and will be responsible for providing nursing care to assigned residents/patients during your shift. Your duties will include coordinating, implementing, and evaluating the resident's/patient's plan of care, ensuring adherence to care standards, managing the environment for safety, and supervising nursing assistants. You will also be expected to follow all center policies and procedures and perform duties as defined by the State Nurse Practice Act.

Responsibilities

  • Provide nursing care to assigned residents/patients under the supervision of the RN Unit Manager or DON.
  • Coordinate, implement, and evaluate the resident's/patient's plan of care.
  • Adhere to the standards of care for the area and manage the environment to maintain resident/patient safety.
  • Supervise the performance of nursing assistants in resident/patient care activities.
  • Follow all Center policies and procedures.
  • Perform duties as defined by the State Nurse Practice Act.

Requirements

  • Licensed in the State as an LPN.
  • Current CPR certification.
  • Ability to use necessary supplies and equipment (feeding pumps, oxygen, treatment trays, medication cart, etc.).
  • Ability to calculate drug dosage.
  • Meet all health requirements imposed by law.
  • Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers, and other center staff.
  • Ability to read, write, speak, and understand English; demonstrates basic computer knowledge and aptitude to learn company applications.
