Responsibilities

• Conduct case consultations and case management for patients.
• Provide psychotherapy and psychosocial education to patients and their support networks.
• Establish initial contact and assess psychosocial and support system needs.
• Recommend updates to care plans to improve patient function and quality of life.
• Collaborate with patients, caregivers, and medical teams to address psychosocial factors affecting health outcomes.
• Maintain thorough documentation for all services rendered according to organizational standards.
• Coordinate care with families, healthcare teams, and community resources.
• Establish relationships with community organizations for referrals and access to services.
• Participate in research initiatives for patient care operational improvements.
• Attend division meetings and continuing education opportunities.
• Provide coverage for urgent needs when other social work team members are unavailable.

Requirements

• Experience in case management and social work services.
