About the position

The Licensed Master Social Worker (LMSW) position at the Office for People With Developmental Disabilities (OPWDD) involves acting as the Residential School Transition Coordinator (RSTC) within the Age Out Unit. This role is crucial for facilitating the transition of individuals with developmental disabilities from residential schools to adult services, ensuring they receive appropriate support and services as they age out of their current settings.

Responsibilities

  • Act as the Residential School Transition Coordinator (RSTC) within the Age Out Unit.
  • Ensure individuals aging out of residential schools receive appropriate OPWDD adult services.
  • Assist with state management and reporting for individuals transitioning from residential schools to OPWDD adult services using Excel, Citrix, and relevant databases.
  • Participate in meetings with agencies, families, residential schools, and developmental staff regarding transitions.
  • Work closely with the Eligibility Team to ensure individuals meet OPWDD criteria for services.
  • Follow the Front Door process, including updating the Eligibility Assessment and Authorization system.
  • Provide information to providers on the Higher Needs Funding process.
  • Visit residential schools for upcoming graduates as needed.
  • Maintain communication with families regarding their child and the placement process.
  • Review Preliminary Adult Services Plan/Behavioral Plan to ensure needs are met by OPWDD Adult Residential Opportunity.
  • Send referrals for residential placement and Day Services to appropriate agencies.

Requirements

  • License and current registration as a Licensed Master Social Worker in New York State.
