About the position
The Licensed Master Social Worker (LMSW) is a vital member of the interdisciplinary team, responsible for assessing psychosocial symptoms of patients and their families. This role involves developing psychosocial outcomes to alleviate identified symptoms and providing psychoeducation, supportive counseling, and casework interventions. The LMSW also participates in the interdisciplinary plan of care, ensuring high-quality psychosocial care and continuity for patients and families.
Responsibilities
- Perform assessments of psychosocial symptoms of patients and families.
- Develop psychosocial outcomes for alleviating identified symptoms.
- Provide psychoeducation and supportive counseling to patients and families.
- Maintain accurate and timely documentation in the patient medical record.
- Participate in the preparation and execution of the interdisciplinary plan of care.
- Attend weekly team meetings to discuss patient care plans.
- Provide psychosocial education and consultation to the interdisciplinary group.
- Participate in psychosocial supervision meetings and training sessions.
- May supervise social work interns and other psychosocial team members.
- Provide bereavement care and facilitate bereavement group activities.
Requirements
- Masters of Social Work from an accredited school.
- At least one year of social work experience in a healthcare setting.
- Knowledge of community resources and psychosocial issues prevalent with terminally ill patients.
- Excellent listening and communication skills.
- Ability to advocate effectively for patients and families.
