About the position
As a Licensed Clinical Social Worker at GuideStar Eldercare, you will provide neurobehavioral health services to residents in long-term care facilities. This role involves collaborating with specialty clinicians to ensure holistic and continuous care for elderly individuals suffering from neurocognitive disorders and other neurological or psychiatric conditions.
Responsibilities
- Provide neurobehavioral health services to residents in long-term care facilities.
- Collaborate with specialty clinicians to deliver holistic care.
- Ensure continuity of care for elderly individuals with neurocognitive disorders.
- Conduct assessments and develop treatment plans for residents.
- Participate in multi-disciplinary team meetings to discuss patient care.
Requirements
- Master's degree in social work.
- Currently licensed as an LCSW in the state of application.
- Bilingual in English and Spanish.
