About the position

The Liaison Representative plays a crucial role in managing the referral process for patients transitioning to assigned SRN facilities. This position involves performing preadmission evaluations, ensuring patients meet admission criteria, and facilitating smooth transitions of care between facilities. The representative collaborates with various stakeholders to remove barriers and enhance customer satisfaction throughout the referral process.

Responsibilities

• Coordinate and manage the referral process to ensure a smooth transition of patients to SRN or PHH.
• Obtain patient assessment information from various sources and transmit data in appropriate systems (4Next/Rehab Checklist/Epic/Allscripts/naviHealth).
• Develop and maintain collaborative relationships with internal and external staff to ensure customer satisfaction.
• Nurture referral source relationships to maintain customer satisfaction.
• Support challenging transfers using SRN/PHH clinical and supervisory resources.
• Confirm and qualify patient admissions as described in the liaison process.
• Ensure admissions adhere to SRN/PHH criteria.
• Consider discharge plan alternatives to meet patients' needs.

Requirements

• Current license in Massachusetts as RN, PT, OT, or SLP.
• Knowledge of IRF LTAC, SNF, and/or Home Care preferred.
• Excellent computer skills with working knowledge of Microsoft Office Suite and ability to learn other software applications; knowledge of Epic is helpful.
• Broad knowledge of various insurances and government reimbursements.
• Strong written, verbal, and electronic communication skills.
• Highly developed interpersonal and relationship-building skills, working with a diverse population.
• Ability to organize and prioritize effectively while working independently and adapting to changing conditions.
• Effective problem-solving skills while meeting demands of all parties.
• Comfortable and enjoys working in a fast-paced environment.
• Ability to make appropriate clinical decisions based on provided information independently.
