About the position

The Lead Transplant Specialist (Registered Nurse) is responsible for coordinating the organ transplant process for both transplant recipients and donors. This role involves maintaining clinical responsibilities, ensuring quality assurance reporting, and managing data related to transplant patients. The specialist will also engage in community education and outreach programs to inform and support patients and their families throughout the transplant process.

Responsibilities

  • Coordinates the organ transplant process for prospective candidates.
  • Reviews referrals and medical records of candidates.
  • Schedules evaluation testing for transplant candidates.
  • Assesses the results of evaluation data with the transplant team.
  • Coordinates the pre-organ transplant evaluation process for hospitalized patients.
  • Maintains medical records and ensures that patient files are complete.
  • Updates database as necessary and coordinates the plan of care for organ transplant patients.
  • Collaborates with the transplant team in planning and implementing a comprehensive pre/post-operative plan of care.
  • Provides support and consultation to co-workers and peers in administering patient care.
  • Ensures all necessary tests are completed and follows up on the results of pending tests.
  • Educates patients and family members about the transplant process and answers questions as necessary.
  • Performs appropriate follow-up of post-operative transplant patients and ensures patient assessment has been completed and documented.
  • Identifies actual/potential patient problems and collaborates with post-operative care members and transplant surgeon.
  • Serves as a liaison between the patient and health care members, and makes referrals to appropriate resources as necessary.
  • Provides information and counseling to transplant patients and significant others, reinforcing information presented by the transplant surgeon.

Requirements

  • Nursing Diploma required, or equivalent combination of education and related experience.
  • Must obtain a Bachelor of Science in Nursing degree within 5 years of employment date.
  • Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
  • 5-7 years of technical experience and 2+ years of leadership/management experience preferred.
  • Master of Science required, or equivalent combination of education and related experience.
  • Current License to practice as a Physician Assistant in New York State required, plus specialized certifications as needed.
  • Having transplant experience is a plus.
