Responsibilities

  • Consistently integrates age specific and culturally diverse concepts into patient care.
  • Identify and advocate for the needs of the patient/families/caregivers within the healthcare system and community.
  • Act as a liaison between the patient/family/caregiver within the medical team and outside agencies.
  • Conduct a comprehensive psychosocial assessment of the patient/family's health care needs.
  • Provide psychosocial services as identified in the patient's comprehensive plan of care.
  • Develop resource networks and excel at resource utilization.
  • Serve as a recognized expert to health care team and/or community on social work practice.
  • Ensure that the clinical practice of all clinical staff conforms to local, state, and federal regulations.

Requirements

  • Master's degree in Social Work or Counseling.
  • 5+ years of work experience in a related job discipline.
  • Current LISW or LPCC license to practice in the State of Ohio.
