Under the direct supervision of the SVP of Behavioral Health, the Behavioral Health Provider (BHP) is responsible for providing trauma-informed behavioral health services at the Zufall Health Center sites. Zufall Health follows the Primary Care Behavioral Health model of integrated care. The BHP provides direct services to patients, families, and groups. They provide brief behavioral health intervention to improve patient health outcomes as well as individual and family counseling, and engages in crisis intervention. The BHP contributes to the patient care team through huddles, case consultation, patient advocacy, mental health education, and collaborative team meetings with clinical and support staff. They identify opportunities for enhancing integrated Behavioral Health Services in the primary care setting and serve as a resource to the rest of the clinical staff. BHP's working towards clinical licensure will participate in weekly clinical supervision (individual or group) with an LCSW who is certified to provide clinical supervision.
