As an Intake Specialist, you will be responsible for matching potential mentors with individuals utilizing clinical principles, monitoring the matching process, conducting telephone intakes, and ensuring the intake process follows internal protocols. You will also coordinate services needed by individuals when placed, present clinical assessments, and maintain relationships with funding agencies. This role requires a commitment to a team-based workplace that puts people first.

Responsibilities

  • Match potential mentors with individuals utilizing clinical principles, meeting with Program Manager, Clinical Supervisor, and/or Recruiter to review potential individual-mentor matches and assist in case assignments; complete all placement paperwork.
  • Monitor the matching process, arrange needed services, and maintain ongoing contact with placement referral sources.
  • Conduct telephone intake with follow-up assessment interviews on appropriate referrals; ensure the receipt of a referral packet and any other necessary documentation for intake.
  • Receive phone inquiries from collaterals regarding potential referrals and conduct follow-up assessment interviews on appropriate referrals and pre-placement visits as necessary.
  • Ensure the receipt of a referral packet and any other necessary documentation for intake; create files for new individuals served and ensure entire intake process follows internal protocol.
  • Secure funding information, which may require verification of insurance coverage arrangements.
  • Coordinate all services that will be needed by the individual when placed as applicable.
  • Present clinical assessments with preliminary treatment plan recommendations to Program Manager, Director, and/or clinical team in consultation with psychiatric or medical consultant as needed.
  • May assist Coordinators in locating appropriate homes when disruptions or respite needs occur.
  • Complete all reporting and mentor supervision requirements consistent with corporate and regulatory standards.
  • Maintain ongoing relationships with funding agencies and develop new resources through marketing presentations.
  • Ensure sufficient number of referrals by marketing services with funding sources.
  • Assist with emergency coverage and crisis intervention support as needed; may fill in for manager during manager's absence or through on-call rotation.

Requirements

  • Bachelor's Degree in Human Services required; Master's Degree preferred.
  • Five years of related experience preferred; required in some states.
  • Experience in psychosocial and psychometric assessments preferred; required in some states.
  • Current driver's license in good standing, car registration, and auto insurance if providing transportation for individuals receiving services or driving for business-related purposes.
  • Current CPR/First Aid Certification preferred; required in some states.
  • Licensure as required by state.
  • Self-motivated and detail-oriented with ability to multi-task.
  • Strong analytical skills with the ability to gather information from different sources.
  • Strong communication skills and ability to build rapport with others.
