Responsibilities

• Act as the primary point of contact for families, ensuring they qualify for services and understand the process.
• Gather necessary documentation from patients and caregivers, maintaining accuracy and compliance.
• Coordinate state assessments and pursue authorizations to ensure timely service delivery.
• Build positive relationships with patients, caregivers, and state agencies through excellent communication and follow-up.
