About the position

The Intake Referral Coordinator position at Personal Touch Home Care is designed for LPNs or Medical Assistants who are looking to transition from traditional hospital settings to a more office-based environment. This role focuses on coordinating patient referrals for home care services, ensuring superior customer service, and maintaining the continuity of patient care. The position offers a stable work schedule from Monday to Friday, allowing weekends to remain free, while providing opportunities for professional growth and development.

Responsibilities

  • Coordinate patient referrals for home care services.
  • Answer phones for intake information and enter data into the EMR system.
  • Complete intake forms, including ICD-10 coding, and route them for admission approval.
  • Forward intake information to PBR for insurance verification and credit approval.
  • Conduct initial nursing insurance authorization on new referrals and maintain a tracking log.
  • Maintain a weekly referral log for all intakes.
  • Communicate challenging cases to the Intake Manager for further review.
  • Assist with in-servicing clinical staff when educational opportunities arise.
  • Perform LPN duties as needed when visits are uncovered.
  • Develop positive rapport with staff members and community resources.

Requirements

  • Valid LPN license in the State of Virginia (preferred).
  • Experience in nursing, particularly in home health care or intake processes.

Nice-to-haves

  • Prior home health care experience.
  • Experience with EMR systems.
