The Inpatient RN Case Manager will coordinate care delivery within a large, award-winning healthcare provider in New York City. This role involves collaborating with healthcare teams, monitoring patient progress, and ensuring effective discharge planning for patients in a non-profit acute care hospital setting.
Responsibilities
- Coordinate care delivery and collect data for quality assessment.
- Act as a liaison with physicians and healthcare teams.
- Review patient discharge needs and monitor patient progress.
- Communicate with third-party payers and conduct case reviews.
- Develop and implement discharge plans for patients.
- Ensure timely care plan development and monitor healthcare processes.
- Participate in performance improvement activities.
- Maintain current knowledge in the field and ensure patient and staff satisfaction.
Requirements
- Bachelor's Degree in Nursing (BSN).
- Several years of case management experience.
- Acute care nursing experience.
