About the position
The Speech Language Pathologist (ST) position is a part-time role focused on providing home health services in the Far Southwest Suburbs of Illinois, including areas such as Frankfort, Mokena, New Lenox, Joliet, Lockport, Homer Glen, Lemont, Palos, Worth, Orland Park, Tinley Park, Oak Forest, Midlothian, Blue Island, Crestwood, and Alsip. The position requires a commitment to quality care, patient education, and effective coordination of services. The Speech Language Pathologist will work as part of a patient care team to ensure optimal health outcomes and will be responsible for adapting practices based on the latest evidence-based literature. This role also includes a focus on leadership, collaboration, and professional development within the healthcare environment.
Responsibilities
- Facilitates the patient and family's right to receive quality cost-effective care.
- Utilizes appropriate resources to respond to situations that may negatively impact outcomes.
- Adapts practice to the latest standards according to evidence-based literature.
- Participates in quality/process improvement initiatives.
- Promotes efforts to reduce risk and improve patient safety.
- Documents thoroughly to support reimbursement and regulatory requirements.
- Assesses patient and family readiness to learn and modifies approach as necessary.
- Provides and adapts appropriate resource material to achieve patient education outcomes.
- Practices as an effective member of the patient care team to formulate an integrated approach to care.
- Recognizes changes in clinical situations and evaluates and implements changes to the plan of care.
- Initiates referrals and discharge planning for assigned patients.
- Participates in interdisciplinary case conferences.
- Demonstrates respect and understanding for peers and participates as an effective member of the patient care team.
- Participates in staff meetings, agency in-services, and educational activities.
- Seeks out opportunities to enhance knowledge, skill, and competency.
- Develops and maintains a restorative relationship throughout the healthcare continuum.
- Promotes patient/family knowledge and confidence to manage self-care.
- Incorporates an individualized approach to the plan of care.
- Demonstrates adaptive communication styles based on individual patient needs.
- Monitors utilization of resources to achieve excellent health outcomes.
Requirements
- Master's Degree in Speech Pathology.
- Licensed in State of Illinois (or eligible for licensure).
- Completion of Clinical Fellowship Year (CFY).
- 1 year of clinical experience or strong clinical affiliation experience.
- Basic Life Support (BLS) for Healthcare Providers certification within 6 months.
- Valid driver's license issued by the Division of Motor Vehicles.
- Basic knowledge of computer use (i.e. Microsoft Word, email).
- Ability to communicate professionally both verbally and in writing.
- Good interpersonal skills and time management abilities.
