About the position

The Inpatient Case Manager at NorthBay Health is responsible for assessing, planning, implementing, monitoring, and evaluating individualized patient care plans across the continuum of care. This role requires collaboration with team members and Social Services to prioritize daily activities and ensure optimal patient outcomes while considering financial and health plan resources. The position demands strong clinical knowledge, critical thinking, and effective communication skills to address the diverse needs of patients.

Responsibilities

  • Assess and reassess patients to address their broad spectrum of needs.
  • Develop and implement individualized care plans in collaboration with healthcare team members.
  • Monitor patient progress and adjust care plans as necessary.
  • Utilize clinical knowledge and problem-solving skills to effect change in patient care.
  • Ensure adherence to department policies, procedures, and quality assurance guidelines.
  • Communicate effectively with patients, families, and healthcare personnel to facilitate care.
  • Demonstrate caring, collaboration, competence, and communication behaviors in all interactions.

Requirements

  • Graduate from an accredited school of professional registered nursing; BSN required.
  • Minimum of 2 years acute clinical nursing experience preferred.
  • Case Management experience strongly preferred or equivalent experience in lieu of a Master's in Nursing with an emphasis in Case Management.
  • Current California state RN license required.
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred.
  • Basic knowledge of personal computers and software for word processing; good keyboard skills required.
  • Ability to enter and retrieve data from relevant computer systems.

Nice-to-haves

  • Knowledge of MCG, Cerner, Medicare rules and regulations, and Conditions of Participation.
  • Familiarity with Joint Commission, Department of Human Services, Patient Safety, and Department of Health organizational standards.
  • Understanding of levels of care including acute care, home care, subacute care, long-term care, hospice interventions, and rehabilitation options.
