About the position

The Inpatient Care Manager Registered Nurse (RN) at Emory Healthcare is responsible for coordinating patient care from admission through discharge. This role ensures smooth transitions of care, facilitates high-quality clinical outcomes, and advocates for patients and families. The RN will work closely with interdisciplinary teams to develop and implement discharge plans, identify post-acute services, and educate patients and families about available resources. This position is PRN, requiring a minimum of four shifts per month, and offers a competitive hourly wage of $48.

Responsibilities

  • Coordinate patient care from admission through discharge.
  • Ensure smooth transitions of care as patients are discharged from the hospital.
  • Facilitate high-quality clinical and cost outcomes.
  • Procure and secure post-acute services for patients.
  • Coordinate and advocate for patients and families with internal and external stakeholders.
  • Identify and address potential barriers to care coordination and discharge planning.
  • Complete thorough admission and psychosocial assessments to inform discharge planning.
  • Attend rounds, care conferences, and care team meetings as part of the interdisciplinary care team.
  • Plan and implement the best possible care plan considering patient/family preferences and limitations.
  • Identify and recommend post-acute services and complete referrals in a timely manner.
  • Ensure discharge plans align with the patient's medically cleared discharge date and projected length of stay.
  • Participate in strategies to reduce unnecessary length of stay and resource consumption.
  • Educate patients/families and the care team about post-acute services and transitions of care.

Requirements

  • Valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
  • Bachelor's degree in nursing preferred or actively being pursued.
  • 2 years of recent healthcare experience required.
  • Ability to meet all quality and productivity expectations and complete yearly competencies.
  • Availability to work at least 4 shifts per month.
