About the position

The Innovations Care Manager (Innovations CM) is responsible for providing proactive intervention and coordination of care to eligible Vaya Health members in Rowan and Forsyth County, NC. This role focuses on ensuring members receive appropriate assessments and services across various health domains, including mental health, substance use, and physical health. The Innovations CM works collaboratively with members, care teams, and community stakeholders to address care gaps and support transitions from institutional settings to community-based care. This position requires travel and is primarily remote, with a requirement to live in or near the specified counties.

Responsibilities

  • Provide proactive intervention and coordination of care to eligible Vaya Health members.
  • Conduct comprehensive bio-psycho-social assessments addressing social determinants of health.
  • Participate in interdisciplinary care team meetings and comprehensive care planning.
  • Support transition planning for members discharged from facilities to ensure follow-up with aftercare services.
  • Utilize Vaya's Care Management software platform for documentation and care coordination.
  • Ensure compliance with HIPAA requirements and maintain confidentiality of member information.
  • Administer various health screenings and assessments to identify member needs.
  • Develop and update person-centered care plans in collaboration with care teams and members.
  • Monitor service delivery and ensure compliance with standards and regulations.
  • Educate members and families on available services and resources.

Requirements

  • Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services.
  • Two years of experience working directly with individuals with intellectual/developmental disabilities (I/DD) or traumatic brain injury (TBI).
  • Experience in Long-term Services and Supports (LTSS) and/or Home Community Based Services coordination.
  • Knowledge of the NC Medicaid program and accreditation requirements is preferred.
  • Ability to complete and maintain all required trainings and proficiencies.

Nice-to-haves

  • Experience with health promotion and community integration programs.
  • Familiarity with the Diagnostic and Statistical Manual of Mental Disorders (current version).
  • Proficiency in Microsoft Office products and Vaya systems.
