About the position

The Innovations Care Manager (Innovations CM) at Vaya Health is responsible for providing proactive intervention and coordination of care to eligible members in Rowan and Forsyth County, NC. This role involves assessing members' needs, developing care plans, and ensuring they receive appropriate services across various health domains. The Innovations CM works collaboratively with a multidisciplinary team to address care gaps, support transitions from facilities, and promote integrated care, all while adhering to compliance and documentation standards.

Responsibilities

  • Provide proactive intervention and coordination of care to eligible Vaya Health members.
  • Conduct comprehensive bio-psycho-social assessments to identify members' needs.
  • Develop and implement person-centered care plans in collaboration with the care team.
  • Participate in interdisciplinary care team meetings and ongoing care management.
  • Support members in transitioning from institutional settings to community-based care.
  • Monitor and document care management tasks and ensure compliance with standards.
  • Educate members and families on available services and resources.
  • Facilitate Care Team meetings to discuss and review member care plans.
  • Engage with community stakeholders to identify and address system barriers.

Requirements

  • Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services.
  • Two years of experience working directly with individuals with intellectual/developmental disabilities (I/DD) or traumatic brain injury (TBI).
  • Experience in Long-term Services and Supports (LTSS) coordination and care management.
  • Proficiency in Microsoft Office products and Vaya's Care Management software platform.
  • Ability to conduct Health Risk Assessments (HRA) and utilize various screening tools.

Nice-to-haves

  • Experience with NC Medicaid program and NC Medicaid Transformation.
  • Knowledge of the Diagnostic and Statistical Manual of Mental Disorders (current version).
  • Familiarity with community integration and supportive housing resources.
