About the position
As an Independent Optometrist at Sam's Club, you will lease space to provide eye and vision care services while maintaining autonomy over your practice. This role allows you to establish your own schedule, control patient fees, and retain all professional fees earned. You will have the option to utilize equipment and support services, depending on state regulations.
Responsibilities
- Provide comprehensive eye and vision care to patients.
- Establish your own practice days and hours.
- Control your patient schedule and fee structure.
- Maintain 100% of exam and professional fees.
- Optionally utilize equipment and administrative support services.
Requirements
- Doctor of Optometry (OD) from an accredited optometry school or valid state ophthalmology license (MD or DO).
- Valid state optometry license.
- General Liability Certificate.
- Professional Liability Insurance Certificate.
- W-9 form.
