The In-Reach Specialist position is designed to engage, educate, and support adults who have a serious mental illness (SMI) or a serious and persistent mental illness (SPMI). The primary goal of this role is to inform these individuals about community-based mental health services and supported housing options. This includes providing information on tenancy support services, rental assistance, and the various benefits associated with transitioning into supported housing. The In-Reach Specialist will work closely with individuals to educate them about the available services, including Medicaid, Special Assistance, and the North Carolina State Plan for Medical Assistance. The position also emphasizes the importance of recovery-oriented activities to prevent relapse and promote wellness among individuals with mental health challenges. The target population for this role includes individuals residing in Adult Care Homes (ACHs) that have been determined to be Institutions for Mental Disease (IMDs). The In-Reach Specialist will assess the interests of these individuals in supported housing, explain the benefits and financial aspects of community-based integrated settings, and facilitate site visits to permanent housing options. Additionally, the specialist will address any concerns from individuals who may be hesitant to transition to supported housing, ensuring they are fully informed about their options and the supports available to them. This role is crucial in linking individuals to ongoing peer support services and educating them about the services covered under various assistance programs. The In-Reach Specialist will also be responsible for establishing rapport with individuals and their families, maintaining effective relationships with community agencies, and ensuring confidentiality in all communications. The position requires travel to various facilities to meet with Transition to Community Living (TCL) members and their guardians, making it essential for the specialist to have a thorough understanding of the recovery process and the resources available within the community.
