About the position
The Hospitalist position at South Shore Hospital's Hospital at Home program involves providing high-quality care to patients transitioning from the hospital to their homes. This role is essential in the innovative care delivery model that South Shore Hospital is implementing, allowing for flexible work arrangements and collaboration with a multidisciplinary team.
Responsibilities
- Admitting patients to the Hospital at Home unit.
- Providing direct patient care.
- Working collaboratively with a multidisciplinary team.
- Participating in Hospital at Home patient acquisition through partnership with the HAH Intake RNs.
- Developing appropriate plans of care and follow-up based on diagnostic assessments and physical examination findings.
- Ordering medications and writing prescriptions according to policies and procedures.
- Participating in daily HAH Interdisciplinary Rounds (IDR).
- Providing oversight of HAH Advanced Practice Clinicians.
- Performing virtual nighttime coverage as needed, on a rotating schedule.
- Participating in clinical onboarding and training of HAH staff.
Requirements
- Medical Degree from an accredited institution (MD/DO).
- Internship and Residency in Internal Medicine or Family Medicine from an accredited institution.
- BC/BE in Internal Medicine or Family Medicine.
- Unrestricted Physician License from the Board of Registration in Medicine.
- Massachusetts Controlled Substance Registration and DEA Registration.
- Active ACLS certification.
- Fluency in English, both verbal and written.
- Ability to learn how to use Epic EMR.
