The Hospital at Home Hospitalist position at South Shore Hospital involves providing high-quality care to patients transitioning from the hospital to their homes. Responsibilities

• Admitting patients to the Hospital at Home unit.
• Providing direct patient care.
• Working collaboratively with a multidisciplinary team.
• Participating in Hospital at Home patient acquisition through partnership with the HAH Intake RNs.
• Developing appropriate plans of care and follow-up based on diagnostic assessments and physical examination findings.
• Ordering medications and writing prescriptions according to policies and procedures.
• Participating in daily HAH Interdisciplinary Rounds (IDR).
• Providing oversight of HAH Advanced Practice Clinicians.
• Performing virtual nighttime coverage as needed, on a rotating schedule.
• Participating in clinical onboarding and training of HAH staff.

Requirements

• Medical Degree from an accredited institution (MD/DO).
• Internship and Residency in Internal Medicine or Family Medicine from an accredited institution.
• BC/BE in Internal Medicine or Family Medicine.
• Unrestricted Physician License from the Board of Registration in Medicine, Massachusetts.
• Controlled Substance Registration and DEA Registration from US DOJ.
• Active ACLS certification.
• Fluency in English, both verbal and written.
• Ability to learn how to use Epic EMR.
