About The Position

The Hospital Pharmacist is responsible for preparing and dispensing medication orders according to physician requests and established protocols. This role ensures safe, appropriate, and cost-effective medication therapies for patients, while also providing drug information to healthcare professionals, patients, and families. The position involves detecting and reporting adverse drug reactions and medication errors, participating in quality improvement activities, and assisting with pharmacokinetics consults and antibiotic appropriateness reviews. Additionally, the pharmacist supervises pharmacy technicians, provides input on their performance, and may act in the absence of the pharmacy director. Adherence to health system policies, safety regulations, and legal compliance is paramount. The role also includes maintaining formulary compliance, managing investigational drugs and controlled substances, and serving as a professional role model and resource.

Requirements

  • Graduate of an accredited Pharmacy program
  • Wisconsin and/or Illinois license required
  • Computer experience, including internet and email
  • Strong accuracy skills
  • Excellent interpretive/analytical skills for pharmaceutical calculations for drug dosage, IV rates, etc.

Responsibilities

  • Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.
  • Provides accurate, adequate, and timely drug information to professional staff, patients, and patient families.
  • Documents activities as needed.
  • Ensures safe, appropriate, cost-effective medication therapies for patients.
  • Detects and accurately reports suspected adverse drug reactions and medication occurrences.
  • Assists other professional staff to identify med errors/adverse reactions.
  • Participates in the quality improvement and medication use review activities of the department.
  • Collects data, conducts quality monitors and inspections, and maintains logs/records and other documentation.
  • Assists with pharmacokinetics consult service/drug dosing per organizational protocol.
  • Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes.
  • Assists professional staff with managing patient's pain.
  • Supervises, directs, and coordinates the daily activities of pharmacy technicians/support personnel.
  • Provides input for pharmacy support personnel performance appraisals.
  • Performs essential duties of the pharmacy director in his/her absence.
  • Adheres to health system policies and procedures.
  • Complies with all requirements related to risk management, safety, security, fire, and infection control.
  • Complies with all applicable federal, state, and local laws, rules, and regulations.
  • Actively participates in the department's quality improvement activities, hospital committees, and department/team meetings.
  • Assists new hires in orientation, education, and training programs.
  • Reviews literature and other materials as assigned.
  • Maintains competence required for current job title/position, including maintaining current professional licensure and completing all competence/skills assessment requirements.
  • Maintains formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug selection.
  • Monitors investigational drug issues and controlled substances to patient care areas & maintains records required by law.
  • Resolves discrepancies as needed.
  • Maintains professionalism and conducts self as a role model, serving as a resource for other retail pharmacy partners.
  • Maintains practice by ongoing professional literature review, continuing education, and self-evaluation.
